Cagrilintide: A Long-Acting Amylin Analogue

The native hormone problem
Human amylin aggregates readily into amyloid fibrils, which makes the native sequence unsuitable as a stable therapeutic or research molecule. Every practical amylin analogue is a redesign around that aggregation problem.
Cagrilintide addresses it with substitutions that suppress fibril formation, combined with a lipid side chain that binds albumin and extends duration substantially.
Receptor pharmacology
Amylin receptors are formed when the calcitonin receptor associates with receptor activity-modifying proteins. Published characterisation of cagrilintide describes activity across amylin receptor subtypes and at the calcitonin receptor itself, which is why it is sometimes described as a dual amylin and calcitonin receptor agonist.
Signalling from these receptors in brainstem regions is the described basis for satiety effects reported in animal work.
Why it appears alongside GLP-1 compounds
Amylin and incretin pathways are distinct, so combining them is mechanistically non-redundant. That is the rationale behind the combination studies in the published literature rather than any claim that one replaces the other.
