Retatrutide vs Tirzepatide vs Semaglutide: A Receptor-Level Comparison

The short version
Semaglutide is a GLP-1 receptor agonist. Tirzepatide is a dual GIP and GLP-1 receptor agonist. Retatrutide adds glucagon receptor activity, making it a triple agonist. Everything else about how they are compared follows from that difference in receptor coverage.

Structural differences
Semaglutide is built on the native GLP-1 backbone with a substitution that blocks DPP-4 cleavage and a C18 fatty diacid chain for albumin binding. Tirzepatide is based on the GIP sequence rather than GLP-1, with its own fatty acid modification, which is why it engages both receptors.
Retatrutide is a further-engineered single chain designed to hold meaningful potency at all three receptors simultaneously. In each case the lipidation is what produces a once-weekly duration profile in published work.
Why the glucagon receptor changes the picture
Glucagon receptor activity is the conceptually interesting part of a triple agonist. Glucagon signalling raises hepatic glucose output and energy expenditure, which historically made it an unwanted target in this class.
The design premise of a triple agonist is that adding an energy-expenditure component alongside strong appetite-axis coverage produces a different overall profile than incretin activity alone. The published research on this remains an active area rather than a settled question.
What the comparison does not tell you
Receptor coverage does not translate directly into a ranking. Potency at each receptor, the balance between them, exposure over time, and study population all shape reported outcomes, and cross-trial comparisons are notoriously unreliable.
For laboratory work the practical implications are simpler: these are three distinct molecules with distinct molecular weights, distinct reconstitution volumes and distinct stability profiles. Treat them as separate compounds, not as versions of one another.
